apply for a service or facility dog

IMPORTANT NOTE: The facility dog application process is under revision. If you are an agency interested in obtaining a facility dog, please contact us directly for more information.

ATTENTION: If you were wounded in the line of duty and are applying for a service dog, DO NOT fill out the financial aid request form. Your application will be sent directly to the Selection Committee for determination of scholarship based on service to community.

Service dog - a dog trained to assist an individual with a disability.

Facility dog - the FSDS trains facility dogs to work with crime victims and otherwise traumatized individuals. We train and certify dogs to work at various levels:

Level I dogs are trained to work within the confines of a single facility, such as an advocacy center.

Level II dogs are trained for crisis response work

Level III dogs are trained to accompany an individual to the courtroom

The FSDS provides fully trained and certified service and facility dogs at low to no cost for those who have served their communities. Examples of some types of service that would qualify under our program include but are not limited to:

  • PoliceMatt and Sarah / Law enforcement / Legal services

  • Fire

  • EMS

  • Military

  • Community service / advocacy centers

  • Ministry

  • Public School Teachers

The application process for receiving a service or a facility dog from the FSDS has several steps.

Step One: Applicants must submit completed application packet to the address below. Incomplete applications will not be considered.

Step Two: Selection Committee reviews applications and makes preliminary recommendations.

Step Three: Applicants who receive clearance from the Selection Committee will be scheduled for an extensive phone interview with a program counselor.

Step Four: Applicants must attend three class sessions to allow staff to observe their interactions with students and current service dogs in training.

Step Five: Applicants will be scheduled for a home study to assist us in better understanding your environment and individual needs.

Please Note that at this time we are only able to place service dogs with local Arizona residents, ages 16 or older.
