apply for a service dog

The FSDS provides fully trained and certified service dogs at low to no cost for those who have served their communities. Examples of some types of service that would qualify under our program include but are not limited to:

• Police
  
  
• Fire
  
  
• EMS
  
  
• Military
  
  
• Community Volunteer
  
  
• Ministry
  
  
• Public School Teachers
  

Applicant information is given to our Selection Committee. This committee is composed from individuals who represent a cross-section of the community. The committee review all applications and makes decisions on which individuals will be granted service dogs through our Puppy Raising Partnership Program. Individuals who are selected agree to participate in selected training sessions and to serve as mentors for our students. The decisions of the Selection Committee are final.
