Evaluator Application

Become a Service Dog Evaluator with the FSDS!

We are actively seeking competent evaluators nationwide. Once registered, your evaluator status will remain current for a period of 3 years, after which time it must be renewed. All necessary forms are available for download on our web site. We will post your contact information on our site for all testing candidates. In addition, evaluators will receive seasonal and training tips regularly in our monthly newsletter. Evaluators may earn up to $50 per test in their spare time.

Qualifications:

You must be at least 18 years or age.
You must affirm that you have no felony convictions on record.
You must submit the application to become a SD Evaluator along with the $50.00 non-refundable processing fee; this is renewable every 3 years.
You must meet at least ONE of the following criteria:
1. Be a Canine Good Citizen (CGC) Evaluator for at least one year; or
2. Be a self-trained team that has graduated from an established SD training program, and have been certified for at least one year (proof required); or
3. Be a self-trained team that has worked together for at least one year, and successfully pass the SD Test with the FSDS; or
4. Be a graduate of a professional Canine Obedience Instructor Training School (proof of graduation required); or
5. Have at least 2 years of documented experience working for a Service Dog Training Program (letter from program required); or
6. Be a graduate of the FSDS Puppy Raising Program

Applications must be submitted by mail. All applicants must review the SD Evaluator Guide and pass a written test to be considered. Incomplete applications will not be processed. The FSDS posts contact information for all of our evaluators on-line in order to better connect test applicants with qualified evaluators.
